To check the payment status of a claim or to verify benefits and eligibility, contact the patient's plan administrator; their contact information is located on the back of the patient's ID or in AFMC's Listing of Plan Administrators.
To check precertification status for groups using AFMC medical management services, call AFMC's CHOICE unit at 800-624-4277.
